Quadriceps tendonitis occurs when the tendon connecting the quadriceps muscle to the kneecap becomes irritated due to strain or overuse, often resulting in knee pain, swelling, and weakness around the knee joint.

Although it can affect individuals of all ages, it is especially common in people involved in sports such as running, soccer, and volleyball, where repetitive stress is placed on the knee.

Anatomy

The quadriceps is a large muscle group at the front of the thigh that plays a key role in straightening the knee. It connects to the kneecap (patella) through the quadriceps tendon, and the patella then connects to the shinbone (tibia) via the patellar tendon. Together, these structures form the quadriceps mechanism.

When the quadriceps muscle contracts, it pulls on these tendons to straighten the knee. The patella acts as a fulcrum, increasing the efficiency and force of this movement.

During daily activities such as walking and running, the knee experiences significant force—up to several times your body weight—placing stress on the tendons and surrounding tissues.

Causes

Quadriceps tendonitis typically results from repeated stress on the knee, especially during activities involving running, jumping, or rapid changes in direction. Overuse is the most common cause, but the condition can also occur in non-athletes.

External factors that contribute include:

  • Improper footwear

  • Sudden increases in training intensity or duration

  • Exercising on hard surfaces

Internal factors may also play a role, such as:

  • Poor flexibility

  • Muscle imbalances

  • Misalignment of the leg or foot

  • Differences in leg length

  • Excess body weight

When the stress placed on the tendon exceeds its ability to adapt, small injuries can accumulate. Over time, this can lead to inflammation or degeneration of the tendon, known as tendonosis in chronic cases.

Symptoms

Pain is typically felt just above the kneecap, especially during movement. The discomfort often worsens with activity and may be accompanied by:

  • Swelling around the tendon

  • Tenderness to touch

  • A warm or burning sensation

  • Stiffness, especially after rest or in the morning

In more severe cases, the pain may limit participation in sports or daily activities.

Diagnosis

Diagnosis begins with a detailed history and physical examination. We assess pain location, movement, strength, flexibility, and overall knee stability.

Imaging may be used when necessary:

  • X-rays help rule out fractures or calcium deposits

  • Ultrasound can detect tendon tears

  • MRI scans provide detailed images of soft tissue and confirm tendon damage

These tests help determine the severity of the condition and guide treatment planning.

Treatment

Nonsurgical Treatment:
Initial care focuses on reducing inflammation and allowing the tendon to heal. This may include:

  • Relative rest and activity modification

  • Anti-inflammatory medications or pain relievers

  • Physical therapy to reduce pain and improve function

Therapy may involve:

  • Ice therapy and electrical stimulation

  • Stretching and strengthening exercises

  • Eccentric training to improve tendon resilience

  • Bracing or taping to support patellar alignment

Custom orthotics and proper footwear may also be recommended to improve lower limb alignment and reduce stress on the tendon.

Education plays a key role in preventing recurrence. Adjusting training routines, warming up properly, and avoiding overuse are essential parts of recovery.

Surgery

Surgery is rarely required but may be considered if conservative treatments do not provide relief. The procedure typically involves removing damaged tissue and stimulating healing in the tendon. In more complex cases, surgical realignment may be necessary.

Most procedures are performed on an outpatient basis, allowing you to return home the same day.

Rehabilitation
