de Quervain’s tenosynovitis is a common condition that causes pain along the thumb side of the wrist and into the forearm. It typically develops just above the base of the thumb and can make everyday hand movements uncomfortable. Fortunately, it is generally straightforward to recognize and manage with the right care.

This guide will help you understand how the condition develops, how to identify the symptoms, and what can be done to relieve the pain.

Anatomy

This condition affects two important tendons that control thumb movement: the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB).

Tendons act as connectors between muscles and bones, allowing movement to occur. The muscles attached to these tendons are located in the forearm and extend toward the thumb.

As these tendons travel toward the thumb, they pass side by side through a narrow tunnel located along the thumb side of the wrist near the radius bone. This tunnel functions like a guide, helping keep the tendons aligned during movement.

Inside the tunnel is a smooth, lubricated lining called the tenosynovium. This layer allows the tendons to glide easily as you move your thumb. When this lining becomes inflamed, it restricts the movement of the tendons, leading to irritation and pain—a condition known as tenosynovitis.

Causes

de Quervain’s tenosynovitis commonly develops due to repetitive hand and thumb activities. Movements such as gripping, pinching, squeezing, or twisting can irritate the tendons over time.

This repeated strain may lead to inflammation and swelling, which interferes with the smooth motion of the tendons inside the tunnel. As the space becomes tighter, friction increases and symptoms worsen.

Certain systemic conditions, such as rheumatoid arthritis, may also contribute to inflammation in the tendon sheath. In some cases, previous injuries may lead to scar tissue formation, further restricting tendon movement.

Symptoms

Symptoms often begin with mild discomfort or tenderness near the thumb side of the wrist. Without proper care, the pain may extend up the forearm or down into the thumb.

As irritation increases, you may notice a catching or squeaking sensation when moving the thumb. This phenomenon, known as crepitus, occurs when the tendons struggle to glide smoothly within the inflamed tunnel.

Swelling may develop near the wrist, and simple tasks such as gripping, lifting, or pinching objects can become painful and difficult.

Diagnosis

Diagnosis is typically made through a detailed physical examination. In most cases, no advanced imaging or testing is required.

A key part of the assessment involves identifying the exact location of pain. This helps distinguish de Quervain’s tenosynovitis from similar conditions, such as intersection syndrome, which affects a slightly different area of the forearm.

One commonly used clinical test is the Finkelstein test. This involves placing the thumb inside the fist and gently bending the wrist. Pain along the thumb tendons during this movement is a strong indicator of de Quervain’s tenosynovitis.

Treatment

Nonsurgical Treatment

The primary goal of treatment is to reduce irritation and allow the tendons to heal. Modifying or temporarily stopping activities that trigger symptoms is essential.

We focus on helping you avoid repetitive strain and maintain proper wrist alignment during daily tasks. Taking regular breaks and reducing stress on the thumb can significantly improve recovery.

A thumb-spica splint may be recommended to immobilize the wrist and thumb, allowing the affected tendons to rest. Anti-inflammatory medications can also help reduce swelling and discomfort.

Our approach includes guided therapy to improve movement patterns, reduce tension, and support long-term healing. In cases where symptoms persist, a cortisone injection may be used to reduce inflammation within the tendon sheath.

Surgery

If conservative treatments do not provide sufficient relief, surgery may be considered. The goal of the procedure is to create more space for the tendons by releasing the tight tunnel.

A small incision is made along the thumb side of the wrist, allowing access to the affected area. The roof of the tunnel is carefully opened to relieve pressure on the tendons.

Once released, the tendons can move more freely. The area heals naturally, with scar tissue forming to create a wider space than before. This procedure is typically performed on an outpatient basis.

Rehabilitation

When managed conservatively, symptoms often improve within four to six weeks. Continued use of a splint may be recommended during recovery.
