Each disc has two main components. The inner portion, called the nucleus pulposus, contains a soft gel like material. Surrounding it is the annulus fibrosus, a strong outer layer made of tough connective fibers.

An annular tear occurs when small cracks or splits develop in the annulus fibrosus. These tears are most common in the lumbar spine. In many cases, they begin internally and progress outward from the nucleus.

In early stages, an annular tear may not cause noticeable symptoms. As the tear worsens, disc material can shift or leak outward. If this material irritates nearby spinal nerves, significant pain can develop.
