Sacroiliac joint dysfunction is a common source of mechanical low back pain. The sacroiliac joint, or SI joint, links your spine to your pelvis. When this joint becomes irritated, you feel pain in your lower back or buttock.

This condition creates discomfort. Surgery is rare. Most people reduce pain with targeted care, guided exercises, and manual treatment.

Anatomy

Your sacrum sits below the lumbar spine. It forms a triangular bone made from fused vertebrae. The SI joints connect the sacrum to the iliac bones on each side of your pelvis. You notice their location as two small dimples at your belt line.

The SI joint is large and stable. It allows only small movements. The joint slides a few millimeters and rotates a few degrees. Strong ligaments hold it together. These ligaments support the pelvis and absorb load from walking, lifting, and standing.

During pregnancy, hormones increase ligament flexibility. This change prepares the pelvis for childbirth. Over time, repeated stress or aging leads to stiffness or arthritis in the joint.

Causes

SI joint pain develops for several reasons.

Common causes include:

• Pregnancy and hormonal changes
• Leg length differences
• Pelvic injury from a fall or car accident
• Repetitive strain
• Arthritis

In car accidents, force from pressing the brake transfers through your leg into your pelvis. This force twists the joint and strains the ligaments. A fall on one buttock creates a similar twisting load.

SI joint dysfunction causes:

• Low back pain
• Buttock pain on one or both sides
• Pain radiating into the groin or down the leg
• Muscle tightness in the buttocks
• Pain when sitting on one side

You may shift your weight when sitting. You may avoid sitting flat due to discomfort.

Diagnosis

Your osteopath starts with a full history and physical exam. You describe your pain, daily limits, past injuries, and family history of arthritis.

The exam includes posture, walking pattern, joint motion, muscle strength, reflexes, and skin sensation. These steps help separate SI joint pain from lumbar disc or hip conditions.

Imaging may include:

• X rays of the pelvis and lower back
• MRI to assess soft tissues and rule out disc injury

MRI uses magnetic waves to show detailed images of joints, muscles, and ligaments.

Diagnosis

General Advice

Do:

• Stay active within pain limits
• Rest when pain increases
• Sleep on your side with a pillow between your knees
• Sit to dress or complete tasks
• Wear flat, supportive shoes
• Take stairs slowly with controlled steps

Avoid:

• Standing on one leg while dressing
• Twisting movements
• Sitting cross legged
• Deep soft couches
• Heavy lifting
• Wide leg movements that strain the pelvis

Nonsurgical Treatment

Conservative care works for most people. Treatment may include:

• Anti inflammatory medication such as ibuprofen or naproxen
• Acetaminophen for pain control
• Manual osteopathic treatment
• Targeted strengthening exercises
• Temporary use of a sacroiliac belt

Osteopathic manipulation restores joint alignment and reduces muscle guarding. One common method used in pelvic treatment is Anterior Posterior Lumbopelvic Manipulation, often called the Chicago technique.

If pain persists, your physician may recommend:

• Cortisone injections to reduce inflammation
• Radiofrequency ablation to reduce nerve pain signals
