A popliteal cyst, also known as a Baker’s cyst, is a fluid-filled swelling that develops at the back of the knee and can sometimes cause knee pain. While often painless, it can become uncomfortable depending on its size and the underlying condition causing it. These cysts are commonly linked to issues such as arthritis, injury, or inflammation within the knee joint.

Anatomy

The knee joint is formed where the thighbone (femur) meets the shinbone (tibia). These bones are covered with smooth articular cartilage, allowing them to glide easily during movement. This surface is lubricated by synovial fluid, which is produced by the joint lining (synovial membrane) and contained within the joint capsule.

A popliteal cyst develops when excess synovial fluid builds up inside the joint and pushes outward through the back of the joint capsule. This forms a small, fluid-filled sac in the popliteal fossa, the hollow area behind the knee. Many people can feel this soft swelling at the back of their knee.

Causes

Popliteal cysts often arise due to increased fluid production within the knee joint. This typically happens in response to underlying conditions such as osteoarthritis, rheumatoid arthritis, gout, or knee injuries.

Damage to the joint capsule can create a weak spot, allowing fluid to bulge outward, much like a tire forming a bubble at a weakened area. Repetitive strain or trauma to the knee can also contribute to cyst formation.

Although these cysts may resemble a blood clot, they are not related—though proper evaluation is important to rule out more serious conditions.

Symptoms

Symptoms are often mild but may include:

  • Aching or tightness behind the knee

  • A feeling of instability or weakness

  • Pain related to the underlying knee condition

  • A visible or palpable lump at the back of the knee

The size of the cyst may change over time, often increasing with activity and decreasing with rest.

In some cases, the cyst may rupture, causing fluid to leak into the calf. This can lead to swelling and pain that may mimic symptoms of a blood clot, making prompt medical evaluation important.

Diagnosis

Diagnosis usually begins with a detailed history and physical examination. In many cases, this is enough to identify the cyst.

If further evaluation is needed, imaging tests may be used:

  • MRI scans provide detailed images of soft tissues and can confirm the presence of a cyst

  • Ultrasound helps determine whether the swelling is fluid-filled or solid

X-rays are typically used to assess underlying joint conditions rather than the cyst itself.

Popliteal Cysts Diagnosis

Treatment

Treatment depends on the severity of symptoms and the underlying cause. Most cases are managed without surgery.

Nonsurgical Treatment:

  • Rest and elevation to reduce swelling

  • Aspiration (draining the fluid) followed by cortisone injection

  • Physical therapy to improve knee strength and flexibility

  • Compression, massage, and electrical stimulation to manage swelling

Managing the underlying condition—such as arthritis or injury—is essential to prevent recurrence.

Surgical Treatment:
If conservative care is not effective, surgery may be considered. The procedure involves removing the cyst and repairing the joint capsule where the fluid escaped. However, recurrence is possible, and surgery is typically reserved for persistent or severe cases.

Rehabilitation

Nonsurgical Rehabilitation:
Recovery may take two to four weeks, depending on how well the underlying condition improves. Therapy may include:

  • Ultrasound and electrical stimulation

  • Soft tissue massage

  • Strengthening and flexibility exercises

  • Iontophoresis for targeted anti-inflammatory treatment

Controlling joint swelling is key to preventing the cyst from enlarging again.

After Surgery:
After surgical removal, most people can return to daily activities gradually. We recommend:

  • Keeping the leg elevated to reduce swelling

  • Using crutches or a cane temporarily if needed

  • Avoiding strenuous activity for several weeks

  • Participating in physical therapy to restore strength and mobility
