Nonsurgical treatment of knee osteoarthritis (OA) focuses on reducing pain and maintaining or improving joint function. Our approach includes medications like acetaminophen and nonsteroidal anti-inflammatory drugs (NSAIDs) to relieve pain and swelling. Newer COX-2 inhibitors may also be recommended.

In recent years, two compounds, glucosamine and chondroitin sulfate, have gained acceptance among many doctors as potential aids in managing knee pain. These dietary supplements, usually taken in pill form, are thought to help protect cartilage cells and may assist in repair.

While some studies support their effectiveness, research still leaves questions, particularly regarding long-term effects.

Anatomy

The main problem in knee OA is degeneration of the articular cartilage, the smooth lining that covers the ends of bones in the joint. This cartilage allows the knee to move freely while reducing friction. Synovial fluid, produced by the joint lining (synovial membrane), keeps the cartilage slippery and nourished.

Hyaluronic acid in the synovial fluid also helps retain water, improving lubrication and cushioning the joint. When cartilage wears away, bone rubs against bone, and small bony growths called osteophytes may form.

Rationale

Glucosamine and chondroitin sulfate occur naturally in joint cartilage and can be taken orally or by injection. These compounds are thought to protect and possibly repair damaged cartilage. Research indicates they may reduce pain, swelling, and tenderness while improving joint mobility.

Laboratory studies show glucosamine is absorbed into the synovial fluid and may stimulate hyaluronic acid production, which helps maintain cartilage health. Both compounds also appear to reduce inflammation, offering longer-lasting results than NSAIDs and with fewer side effects. Maximum benefits are typically observed after 8–12 weeks, though improvements may continue beyond treatment.

Preparation

Before starting treatment, gather information from reputable sources like the Arthritis Foundation. Talk to your doctor, as not all forms of arthritis respond to these supplements. Glucosamine and chondroitin sulfate are one part of a comprehensive approach to managing knee OA.

Procedure

Doctors usually prescribe glucosamine at 500 mg three times daily or 1,000 mg twice daily. Chondroitin sulfate is commonly given at 1,200 mg daily. Dosages may be adjusted for obesity or other factors.

Complications

Most patients tolerate these supplements well. Mild gastrointestinal issues are the most common complaints, and rare side effects may include nausea, headache, heartburn, and mild swelling. Patients on multiple medications, diabetics, children, pregnant women, or those on blood thinners should consult their doctor before use.

After Care

Patients often continue to benefit after stopping the supplements. Effects may take longer to appear than NSAIDs but tend to last longer. While glucosamine and chondroitin sulfate may help protect cartilage, they are most effective when combined with other strategies for knee OA.

Rehabilitation

Optimal management includes:
