Tennis elbow, medically known as lateral epicondylitis, is a condition that affects more than just athletes. While it is often associated with tennis players—particularly due to the repetitive backhand motion—it can develop from many everyday activities. Tasks such as painting, using hand tools, gardening, or operating machinery can place repeated stress on the forearm muscles and lead to this condition.

Any repetitive motion that overloads the muscles and tendons of the forearm may result in irritation and discomfort around the elbow.

Anatomy

Tennis elbow primarily affects the outer portion of the elbow, specifically the lateral epicondyle—a bony prominence where key forearm muscles attach. These muscles are responsible for extending the wrist and fingers and are connected to the bone through a shared tendon.

Tendons are made up of strong collagen fibers arranged in parallel bundles. This structure allows them to withstand significant pulling forces during movement. Whenever you grip, lift, or extend your wrist, these muscles contract and place tension on the tendon attached to the lateral epicondyle.

Over time, repeated strain from these actions can overload the tendon, leading to irritation or degeneration.

Tennis Elbow Anatomy

Causes

The most common cause of tennis elbow is overuse. Repetitive activities that involve gripping, lifting, or wrist extension can gradually strain the tendon. This is not limited to sports—routine tasks like hammering, lifting heavy objects, or pruning can also contribute.

In some cases, the body initially responds with inflammation, a process known as tendonitis. However, many cases of tennis elbow involve a different process called tendonosis. Instead of inflammation, the tendon undergoes gradual degeneration due to repeated micro-injuries.

With ongoing strain, the collagen fibers within the tendon begin to break down. The body attempts to repair the damage by forming scar tissue, but this tissue is weaker and less flexible than healthy tendon fibers. Over time, the tendon becomes thickened, less resilient, and more prone to pain and dysfunction.

Symptoms

The most noticeable symptom is pain and tenderness on the outside of the elbow. This discomfort may extend down the forearm and sometimes reach the back of the hand or fingers.

The forearm muscles may feel tight, sore, or fatigued. Pain often increases with activities that involve wrist movement or gripping. Actions such as lifting objects, turning the palm upward, or holding items with a straightened arm can aggravate symptoms.

Even simple daily tasks—like picking up a carton of milk or opening a door—may trigger discomfort. In some cases, the elbow may feel stiff and difficult to fully straighten.
