Sciatica describes pain that starts in the buttock and travels down the leg. Most cases result from irritation of spinal nerves in the lower back. In some cases, the source lies outside the spine. Piriformis syndrome is one such cause.

Piriformis syndrome develops when the piriformis muscle irritates the sciatic nerve. The condition causes pain but rarely requires surgery. Most people improve with structured conservative care and guided rehabilitation.

Anatomy

The sciatic nerve forms from lower lumbar spinal nerves. It exits the pelvis through the sciatic notch.

The piriformis muscle begins inside the pelvis and attaches to the sacrum. The sacrum connects to the pelvis at the sacroiliac joints. The muscle then attaches to the greater trochanter of the femur.

The piriformis acts as an external rotator of the hip. It turns the leg outward during movement. The sciatic nerve travels beneath, and sometimes through, this muscle. When the piriformis tightens or swells, it compresses the nerve and produces sciatic pain.

Causes

Sciatic symptoms arise when the sciatic nerve becomes irritated. In piriformis syndrome, muscle spasm often triggers this irritation. Tightening of the piriformis compresses the nerve against the pelvis.

Direct trauma, such as a fall onto the buttock, can injure the muscle. Bleeding within the muscle forms a hematoma. Swelling increases pressure on the nerve.

As healing occurs, scar tissue may replace normal muscle fibers. Scar tissue lacks elasticity. The muscle remains tight and continues to irritate the sciatic nerve.

Causes of Sciatica Symptoms

Symptoms

Piriformis syndrome causes pain along the back of the leg. Symptoms usually affect one side. Pain may extend toward the foot.

Common features include:

• Buttock pain
• Pain radiating down the leg
• Discomfort with prolonged sitting
• Tingling sensations
• Rare weakness in the leg or foot

Many patients avoid sitting flat. They shift weight away from the painful side.

Symptoms Sciatic Nerve Pain

Diagnosis

Evaluation begins with a detailed history and physical examination. You describe pain pattern, aggravating positions, and past injuries.

Assessment includes posture, walking pattern, spinal motion, and hip mobility. Strength, reflexes, and sensation are tested to rule out lumbar disc involvement or sacroiliac joint dysfunction.

Imaging may include:

• X rays of the lumbar spine and pelvis
• MRI to evaluate soft tissue and exclude disc pathology
• MR neurography to assess nerve irritation
• Bone scan when stress, infection, or inflammatory disease is suspected

The most reliable confirmation involves a diagnostic injection into the piriformis muscle under imaging guidance. If anesthetic relief occurs, the diagnosis is confirmed.

Treatment Options

Nonsurgical Treatment

Conservative management is the first approach.

Treatment options include:

• Anti inflammatory medication
• Structured physical therapy
• Targeted stretching exercises

Stretching the piriformis reduces pressure on the sciatic nerve. Strengthening hip stabilizers improves pelvic alignment.

If pain persists, image guided injections may be used. Local anesthetic relaxes the muscle. Cortisone reduces inflammation. Botulinum toxin temporarily relaxes the muscle to reduce nerve compression. Effects last several months while rehabilitation corrects muscle imbalance.

Surgery

Surgery is reserved for severe, persistent cases. Procedures release the piriformis tendon or muscle to reduce nerve pressure. Most surgeries are outpatient procedures. Recovery focuses on gradual mobility and strengthening.

Rehabilitation

Rehabilitation typically lasts six to eight weeks.

Treatment includes:

• Heat therapy to reduce spasm
• Ultrasound for deep tissue heating
• Soft tissue mobilization

• Posture training
