Arthritis of the lumbar facet joints is a common source of low back pain. Facet joints sit at the back of your spine and connect one vertebra to another. Smooth cartilage covers the joint surfaces. When this cartilage wears down, arthritis develops and movement becomes painful.

Anatomy

Your spine contains 24 vertebrae stacked to form the spinal column. This column supports your body and protects the spinal cord. The back portion of each vertebra forms a ring. When stacked, these rings create the spinal canal, which surrounds the spinal cord.

Each spinal level has two facet joints, one on each side. These joints guide movement as you bend forward and backward. Cartilage covers the joint surfaces and allows smooth motion with minimal friction.

Causes

Facet joints move with every spinal motion. Over time, pressure and wear thin the cartilage. As spinal discs lose height with age, the space between vertebrae narrows. This change increases load on the facet joints.

Past injuries also increase risk. Fractures, ligament damage, or disc problems alter alignment and stress the joints.

Your body responds to extra stress by forming bone spurs. These spurs enlarge the joint, a process called hypertrophy. When cartilage wears away, bone rubs against bone. The joint becomes inflamed and painful.

Symptoms

Facet arthritis causes mechanical low back pain. Pain often increases after rest or sleep. Bending backward or to one side often worsens symptoms on the affected side.

Pain usually stays in the low back and buttocks. It sometimes spreads into the thighs. It rarely travels below the knee.

If bone spurs irritate nearby nerves, you may feel:

Diagnosis
